This is the Guru Yoga Prayer from Karma Kamtsang Foundation Practices (Ngöndro).

A prayer that I have repeated more times than I can count, I first translated it back in the early ’70s. I have revised it many times since, as my understanding of Tibetan idioms and my understanding of this prayer’s meaning matured over the years.
